My friend and fellow custom Rivendell owner who shall remain unnamed unless he chooses to identify himself, has something like that on his bike but with a 30 bailout gear.
He makes it out of 2x 10-speed Ultegra casettes, a junior 14-25 and a 12-30 (I think). IIRC the top 7 gears are from the 14-25 and the big gears on the spider in the back are from the 12-30. He keeps a lot of gears in the 70s and 80s gear inches that way. Runs with a 50-40-30 Record in the front. It's an expensive solution though, I think he said he spends like $120 or $130 putting it together. I personally could not wrap my mind around the expense, but if you know exactly what range you want to be in for cruising, it's one way to go.
