It's a rails counter cache for the note comments:

So unless it has managed to get our of sync somehow it should accurately
represent the number of comment records that exist.
It will include hidden comments though, which the view will suppress unless
you're a moderator.
Possibly there is also some weirdness around whether the initial comment for
opening a new note is shown and/or counted but I'm not sure offhand what the
status of the work to adjust the schema for that is.
