Very strange. I would look closely at the premissions currently on your log
file.

Note also that there is a patch available for a problem with logging in 2.13.1.

That sort of thing is usually caused by lost Stop packets from your NAS.

Can you confirm with the help of your log files whether you got a Stop for the
last session the broken user was connected to?

Simultaneous-Use is a check item, not a reply item, so you need, for example:

username Password=1234,Simultaneous-Use = 1
        reply-attr1=xx,
        reply-attr2=xx,
        ....

You might also want to look at the new DefaultSimultaneousUse discussed in the
2.13.1 patches README.
