This is what the R-patched and R-devel versions of R are for. Please see the R FAQ. Much of the base package is implemented by the compiled code in the R executable/DLL. update.packages() can only update packages on the selected repositories. The standard packages are never there. Updates to the recommended packages which are part of the tarballs may or may not be: for example at the moment there are several updates which are only available as part of R-devel snapshots.
