I just don't see how this particular test is going to be 'very
wasteful' in the scope of qpsmtpd -- an app that will spend most of
it's time waiting on network io or backend (spamassassin) io.

Any time you spend doing useless system calls isn't available for spamassassin or clamav. Run strace against qpsmtpd (at least forkserver variant) some time and you'll see how often config files are opened.
