You can't use async/* plugins with regular qpsmtpd. You can only run
them under qpsmtpd-async.

ah, that's enlightening... qpsmtpd-async doesn't appear to be shipped
with the debian package. At any rate we've already tested our product
with qpsmtpd-forkserver for long enough that I think I'll just revert to
the non-async version.
Thanks for clearing this up, it was difficult to troubleshoot under the
assumption that it was supposed to work :)
