Can anyone shed some light on what might be causing this?

I'm running QPSMTPD 0.40 release, no tweaks and straight qpsmtpd, not -forkserver or any of the other variants.
Any clues would be helpful, I'm not certain if this is causing lost mail or not - it's too early to tell. I really don't want to disable greylisting, it's far too effective against spam but I'll have to if I can't figure out what this is (or at least be assured it's harmless).
