Are you seeing anything in /var/log/dovecot.log ?

Also, are you using SSL/TLS ?

Is this when you're sending email, a Submission issue, or simply when
you have Outlook open on the account, an IMAP issue?

In either case does it correspond to an upgrade of either Qmail or Dovecot?
