try disabling tcpserver dns lookups with the -H switch - that made a world 
of difference for us

from my /var/qmail/supervise/qmail-smtpd/run file:

exec /usr/local/bin/softlimit -m 2000000 /usr/local/bin/tcpserver -H -v -R 
-l 0 -x \
   /etc/tcp.smtp.cdb -c "$MAXSMTPD" -u "$QMAILDUID" -g "$NOFILESGID" 0 \
   smtp /var/qmail/bin/qmail-smtpd 2>&1
