I have a feeling I'll get laughed at, but here goes.
I'm not using svscan.  This is what I have in (*gulp*) inetd.conf:

smtp    stream tcp nowait qmaild /var/qmail/bin/tcp-env tcp-env   
/var/qmail/bin/qmail-smtpd
