Hmm. I can't see where that "Subject:" is coming from. Do you have a
wrapper around qmail-inject, or does it point to something other than
the real qmail-inject?

What's the output of:

  ls -l /var/qmail/bin/qmail-inject 
  file /var/qmail/bin/qmail-inject
