My /var/qmail/rc file reads:

exec env - PATH="/var/qmail/bin:$PATH" \
qmail-start "`cat /var/qmail/control/defaultdelivery`" \
accustamp | splogger qmail &

defaultdelivery reads: ./Maildir/

I have killed all the qmail-* processes (not tcpserver smtpd) and loaded 
qmail-start from the command line manually.  I used:

exec env - /var/qmail/bin/qmail-start ./Maildir/ &

... and a ps would show an immediate exit 111.

Not backrounding the process while using exec env will log me out of the 
terminal.  There is nothing interesting in syslog.
