Not at all, as long as you keep it to yourself.
If you modify qmail (or any GPL'd product) and
you want to distribute the results as your own
prduct, you must put the new product under the
GPL with all sources (both qmail's and your
own) available to any who request it.
That being said, if you make any improvements
upon the existing product that you feel others
may find useful, you may want to support the
spirit of Open Source and make your patches
available for others to use.
