Is mutt configured to use sendmail, or qmail-inject? If sendmail,
have you replaced the sendmail (in /usr/lib or /usr/sbin) with a
symlink to /var/qmail/bin/sendmail?
I don't think either qmail-inject or qmail's sendmail clone ever exits
127. But maybe mutt does, if something goes wrong after it forks to
run qmail-inject or sendmail?
You could try tracing the system calls of mutt and its children, if
you're on a system supporting this.
