What you describe doesn't explain the duplicate emails. Qmail-Scanner
doesn't like what spamd is returning and after trying three times will
do a "temp fail" to the SMTP sender and will not deliver the message.
Later on the sender tries again and probably fails again - end result
should be no email delivered - not duplicates.

As far as the spamd issues go, it sounds to me like you have
qmail-scanner set to call spamc with a maxsize option that is larger
than the server - that is *not* a good idea and definitely needs fixing
- spamc and spamd have to agree over maxsizes. i.e. when you configure
qmail-scanner, use the " --sa-maxsize" option and ensure it matches what
spamd is set to.
