Could be related to your "nofiles" - group. My /var/spool qscan looks like this

Try to give the group qscand read access to this /var/spool/qscan directory...
To test if qmail-scanner is able to scan this directory with clamdscan , simply
test :
clamdscan /var/spool/qscan/tmp/
