Mattew, did you read the bit in the documentation about ensuring you run
"qmail-scanner-queue.pl -z" every night (and after every AV update)?
Doing so will clean out old files which *do* occur as part of the normal
error conditions seen with a mail server (e.g. SMTP client dropping
connection part-way through session). The first time you run "-z", it
will probably take a very long time to finish, but then once per day
should be very fast.
