Have you looked at contrib/qscan-spam-to-users.pl within standard Qmail-Scanner-2.XX? This script (run hourly or more often from cron) will move quarantined spam into a Maildir structure under (new account) qspam. Then you simply log into qspam via IMAP to see all spam separated into one subfolder per recipient.
