reformime reporting a "write error" of any description implies a major
problem. Is that happening on every message, or just one in a thousand?

You say mail is flowing through fine, but that error STOPS ALL MAIL.
Either it's an error that only occurs once in a blue moon, or you don't
actually have Qmail-Scanner being called at all - which would then
explain why you're not seeing any Spam markup.
