I believe qmail-scanner does not scanner messages,,, what scans messages is either perlscanner,spamassassin and clam antivirus that is if ur using clam.

The company that i work for is using qmail-scanner on sever "mailgateways" the issue is when qmail-scanner goes to scan larger messages with attachments. Hi I don't think qmail-scanner will be the problem... Maybe the trasmission of that 2 Mb size messages is slow...
You can try to send a message of that size, localy (from the same LAN of the server) and see if it is slow.
