With Spamassassin 3.x the header displaying the spam scores change from 
'hits=' to 'score=' but qmail-scanner looks for 'hits='... so you have 
to change the pattern:

the line to change looks like this (only have 1.22 here, I suppose it's 
the same problem with 1.25)

if (/^X-Spam-Status: (Yes|No), hits=(-?[\d\.]*) required=([\d\.]*)/) {

change it to

if (/^X-Spam-Status: (Yes|No), score=(-?[\d\.]*) required=([\d\.]*)/) {
