Yes - it means qmail-smtpd started receiving an e-mail, and something happened half-way through that made that connection drop. Unfortunately for qmail-queue/qmail-scanner, they have already been called, so it's up to them to clean up the mess - not qmail-smtpd. qmail-smtpd "tells" qmail-queue/Q-S that this is a broken transaction by not setting the envelope variables - so they cleanup and stop.
Things like network glitches/drops and poorly written SPAM smtp engines can cause such events. Also, perhaps something like an overloaded firewall in front of your mail servers might do it too - basically anything that causes network drops.
