Actually it works on remote ip address, it doesn't care if it is
client relaying his mail through your server, or if it is another
server delivering a mail to your local domain. So it should work for
you.
Alternatively, you may want to check the another patch for ucspi-tcp:
http://linux.voyager.hr/ucspi-tcp/ - you can limit not only by number
of connections from one IP address, but also on system load (for
example, do not accept connection from some range of addresses if the
load is above 10, while still accepting them from some other
addresses) and more.
All options can be combined (they're specified in standard tcpserver .cdb file)
