Sorry if this is obvious and you have done it, but have you noticed the warning message in quarantine-attachements.txt, which says :

  # ******
  # REMEMBER: run /var/qmail/bin/qmail-scanner-queue.pl -g after
  # this file is modified
  # ******

Because the txt is only used to generate a db binary file, which is what Q-S
uses in the end, and this step has to be done also the first time.
(IIRC you are asked to do it to test the suid capabilities of perl after
running configure).
