I tried qmail-scanner and I´m stuck with the problem that outgoing email is not filtered. I have a gentoo box, on my first install I install everything from portage (ebuilds on gentoo tree) and the second time on a clean system from sources at http://www.qmailrocks.org.

My system use courier-imap for imap/imaps, and users are handled by vpopmail (currently 5.4.6).

My vpopmail configuration options were:
./configure --enable-logging=p --enable-auth-module=mysql
--disable-passwd --enable-clear-passwd --disable-many-domain
--enable-auth-logging --enable-mysql-logging --enable-valias
--disable-mysql-limits --enable-roaming-users --enable-apop
--enable-relay-clear-minutes=30 --enable-tcpserver-file=/etc/tcp.smtp
--enable-domainquotas

And my tcp.smtp is:
# Qmail-Scanner disabled for mail from localhost, relay allowed
127.0.0.1:allow,RELAYCLIENT="",RBLSMTPD="",QMAILQUEUE="/var/qmail/bin/qmail-queue"

# Qmail-Scanner enabled (virus only) for mail from local network, relay allowed
196.12.161.:allow,RELAYCLIENT="",RBLSMTPD="",QMAILQUEUE="/var/qmail/bin/qmail-scanner-queue.pl"


# Qmail-Scanner enabled (virus and spam) for mail from external internet, relay denied
:allow,QMAILQUEUE="/var/qmail/bin/qmail-scanner-queue.pl"


All the machines on my lan use the ip 196.12.161.x, also have defined QMAILQUEUE in /var/qmail/supervise/qmail-smtpd/run and /etc/profile
