This is usually done by qmail-smtpd before it gets to Q-S.  The way you
do it depends on how your system is configured.  I use vpopmail as the
final delivery agent so I have patched my qmail-smtpd with the chkuser
patch so that it consults vpopmail to determine if the account exists
and is active before accepting the message.
