We've had the same problem as well.. I have yet to determine what is
causing it.  It doesn't appear to have popped up until we started using
qmail-scanner, so I initially thought that the problem may lie there... I
have not found anything to prove this either way ...  I'm unsure where
these are sourcing, but they are definitely an annoyance...

Squirrelmail, Evolution, and Thunderbird don't seem to have a problem

The problem is determining validity ...  I don't believe it's valid from
an RFC perspective but many mailers have been written to just accept some
missing information from mail and not create errors...

We previously used a windows based product that had a host of problems
....  Moving to qmail was the best move we ever made...  :)
