Really nobody knows? "grep "hits=?" /var/spool/qmail/qmail-queue.log |wc -l" shows about 3300 =(

I think that it is produced by a messages bigger than 300 kb (or something like that), SA skip the message but nothing is write to qmail-scanner log, neither to spamd log.

11:27:05 2 SMTP-927(apo135.usc.urbe.it) {S.0000070509} received, 867545 bytes
As you could see the message had an 850 kb attachment. I don't know if it is possible to change this threshold in spamassassin, but maybe it isn't useful to do so.
