There is the excellent patch by Russell Nelson that blocks mails based
on base64 signatures in MIME parts. Get the patch from:

http://www.qmail.org/qmail-smtpd-viruscan-1.3.patch

and see the short documentation at the start of the diff for usage
instructions.

But notice: This patch does block mails based on content not file types.
So you can't say: "block *.exe files" but "block attachments that start
with TVqQAAMAA in base64". We reject a lot of virus mails before they
even come near the virus scanner.
