This is really strange, so I have some questions:
How are you starting up qmail, and how do you call qmail-scanner?
Does qmail-scanner-queue.pl looks like this:
-rwsr-xr-x 1 qscand qscand 95007 Feb 10 18:21 /var/qmail/bin/qmail-scanner-queue.pl
Who is the owner of /var/spool/qmailscan/qmail-queue.log?

Hi Salvatore. After a tip from yunakaof, the problem was easy to diagnose and resolve. My build of Perl did not contain the suidperl binary. Although perl was chmod-ed as 4711, qmail-scanner-queue did not seem to like running as scand under the perl binary
I downloaded a fresh copy of Perl and made this with suidperl included. Building qmail-scanner-queue correctly picked up suidperl. I am now back in business, this time not as root.
I am assuming that this is the correct way to have built perl as this is discussed in http://qmail-scanner.sourceforge.net/FAQ.php. Perhaps this could be highlighted for dimwits like me .....
