I didn't follow this thread to closely, and I'm kind of shooting in the dark here, but when I install the clamav-freshclam package on debian, it asks if I want to notify clamav-daemon when there's an update. Something about "choosing No here will cause clamav-daemon to not notice updates for several hours." So maybe your freshclam isn't telling clamd about the new updates, and clamd takes a while to notice? I don't know how that situation can be fixed on a non-debian distro; I've never had the problem myself. But then again, you said you tried restarting clamd, so I don't know if that's the reason.
