First of all, let me say I'm not positive that I understand exactly the problems, you're having. I'm gathering that you do not have your customer base on a small set of subnets and that rather, they are spread through out the internet. To offer them mail relaying you have opened relaying to a very large portion of the world. If this is the case, I believe first of all you need to drastically rethink your methods. Here are two ways of dealing with customers not residing in a small pool of IPs: A. implement SMTPauth and don't allow relaying from any subnets at all. B. Your radius server may allow you to log your customer's IP's to a mysql database. You then could use the mysql patch for tcpserver and then in your tcpserver .cdb file disallow relaying to the world. We use this solution for our customers. We use FreeRadius for this purpose.
Lastly, I think you should rethink using qmail-scanner for spam purposes, it, in my opinion, doesn't provide the flexibility necessary for SPAM filtering.
