Check this link out for a qmail-scanner patch .. it allows for an intermediate quarantine of suspected spam as well as deletion of known spam: http://xoomer.virgilio.it/j.toribio/qmail-scanner/
I've only recently applied the patch myself and can't personally vouch that it works yet -- still testing it out.
