Are you asking for help as to why SA isn't adding all its headers and why its not rewriting the subject?
If so, I see you're using qmail-scanner. I just had this problem. Edit your spamc options in qmail-scanner-queue.pl and remove the -c option. This, if I understand correctly, for fast SA, whereas you want verbose SA. Regardless, this seemed to solve my similar problem.
