I have 3 questions:
If I am using the wrapper do I call the wrapper from the tcp.smtp file like this:
:allow,QMAILQUEUE="/var/qmail/bin/qmail-scanner-queue"
or do I call the perl script directly?

Set it up just like the manual says to. Leave it called from tcp.smtp and you will be fine. I run this on OpenBSD 3.3 and a LWQ installation and it works like a champ. That's the point of the QMAILQUEUE patch; look for the variable to be set and execute whatever is set as the variable.
