But I was trying to avoid patching my qmail installation, so it would be
good if qmail scanner had a way to simply bounce back a note.

This is not a spam related concern, in which case the patch would be much
more efficient, but is only a security safeguard against mail flooding the
whole company, and patching qmail soesn't seem justiified for that.

Is there no way qmail scanner can be used?

Use qmail control files to do it.  Add an entry to
/var/qmail/control/badrcptto file.
