2 options: 1) control-click on the QGIS app and select Open and give permission to open QGIS or 2) after having double clicked on the QGIS app and being refused Apple > System Preferences > Security & Privacy and give permission to open QGIS
Either way, you will only be asked once, thereafter QGIS will always start with a double-click or whenver you double-click a QGIS file such as a .qgz or .qgs Project file.
Note, however, that when you download a new version you will have to go through the above steps to authorize it...

The options are then only to move it to the trash or click "Done". Neither of the options starts the programme.
