If you are using QGIS 3.16 then this reference is supposed to be the latest

I have had a similar problem in the past but it generally works if you set the 
Georeferencing SRS in the Georeferencer Transformation Settings to the map’s 
current CRS (shown in the bottom left corner of the map).

I have a jpeg image that I georeferenced using the steps mentioned in the 
following link. At the end, the image does not fall in the right place. As 
shown in step13, a modified image is created and saved in the folder. This 
modified image layer name appears in the layers panel of QGIS but the image 
does not appear on the map.
