Mmm, another thing: I also know that the georeferencer window in Windows sometimes is open, but behind the main window (somewhere), apparently it does not always open in front. Can you check that? And else I think GeoReferencer plugin saves it state itself, so other then either attache a second monitor and move it back. OR Start QGIS with a fresh configuration like: qgis.bat --configpath c:\foo (sorry I'm not on Windows, but that is how you do it in Linux...)
