you could call a simple bash script in a git hook that syncs your
MANIFEST.in with your .gitignore. Something like:
echo -n "exclude " > MANIFEST.in
cat .gitignore | tr '\n' ' ' >> MANIFEST.in
echo "graft $(readlink -f ./keep/this)" >> MANIFEST.in
