Having only 10 hot spots from which to choose in VoiceOver is one of its unfortunate limitations. That said, you can functionally increase this by using the Activities feature of VoiceOver. Within the Activities category of the VoiceOver Utility, creating a new activity based on applications allows you to define VoiceOver behavior when that application is active. Checking the Hot Spots checkbox offers 10 hot spots specific to any application you define. So, you can have 10 spots for each unique application. Of course, a utility like Keyboard Maestro is practically indispensable for true screen macros but having 10 dedicated hot spots for any given application without having to script for it is nevertheless handy.
