I am in the middle of project where I'm using FDF-formatted data, out
of a MySQL database, to merge into a set of pre-defined PDF templates.
We're using CutePDFPro on the Windows workstations to mark up the PDF
templates, and a PHP app on a LAMP server to dynamically generate the
FDF data and use a commandline tool, pdftk to merge, flatten and
compress the resulting PDF documents. I'm basing some of the work on
the O'Reilly book, PDF Hacks. The tools are Open Source, and can be
accessed from http://www.pdflabs.com/ Perhaps you'll find something
there that will help.
