It makes a checksum of the header cache struct, if this changes then
the cache is automatically invalidated.

Do you have a log for a build showing this please? It's odd - though
I don't think it's related to dropping USE_GROFF (groff would have the
same problem as it just takes man5/muttrc.5 as input, so if that's
empty, there won't be any output from groff either).
