I don't believe there is just one source for this problem - as a result
what seems like many users reporting a lot of instances of the same 
problems is in fact, a few different problems which produce the same
symptoms.

If you want to check if this is your issue. Attached is a zipped copy of
the plugin HLS.pm file.  This file has been modifeid to increase the
timeout and generate log message if GET response times exceed 400ms.  

All you have to do is download the attached file, unzip it, stop LMS,
backup current HLS.pm file, replace with unzipped file and then restart
LMS.  The error message in the log produced at the same time as your
problem will indicate if DNS is the problem.
