Oh, I'd just been stupid.  In the <remotes> section of the xml file I
had called the remote 'SR6' but In the <profiles> section (that maps a
remote's ir codes to one of the controller's buttons), and the <menus>
section (that creates menu entries) I had called it 'NAD_SR6'.  The line
in question was #32 (<remote ID="NAD_SR6"> - this is the revised
version).  It doesn't matter what you call the remote in SqueezeIR but
you have to be consistant when referring to it.  I have very little
error-checking code so if the xml is flaky then it just doesn't work. 
I'm thinking about creating a SqueezeIR.log file seperate from the main 
SqueezePlay log to enable people to spot errors more easily.
