I'm having a few issues with custom scan

1. Custom scans are taking place without intervention by me at
unscheduled times.  I assume because something changed in my music
library, but I'm not sure; I don't see anything in the scan logs except
a scheduled scan from the morning before.  How do I turn off this
behavior so that scans are only started when scheduled or when I start
them from the web interface?  Custom browse menus are unusable during
the scan, so it's very annoying.

2. I turned on debug logging for custom scan, which did log output to
the server log for the next scan, but there was no output for subsequent
scans.

3. Custom scans are taking several hours.  Is this normal?  I currently
have about 25000 flacs, and that number will probably double.

4. In the "X by Composer" menus the last composer alphabetically is
always missing in the controller menu, though they all show up fine in
the web interface (poor Ellen Taaffe Zwilich, never to be heard again.) 
Is this a known bug?
