I've installed multipath-tools and multipath-tools-boot instead and started
both.

dpkg -l |grep multipath
ii multipath-tools 0.4.8-14+lenny2 maintain
multipath block device access
ii multipath-tools-boot 0.4.8-14+lenny2 Support
booting from multipath devices

lsmod |grep multi
dm_multipath 13160 0
dm_mod 53546 13 dm_mirror,dm_log,dm_snapshot,dm_multipath
scsi_dh 4624 1 dm_multipath
