Yes the government is implementing a health records bases on open source program whose trials had begun a year ago according to my knowledge. There is also a documentary on this subject by Rajyasabha TV . We were the finalists in Smart India Hackathon 17 and one of the problem statements was based on the same issue , but by then NIC already had a project going in trial at AIIMS hospitals , and plans to make it Nationwide also maybe giving it the "shield of Aadhaar* .
