Toolbars and dialog options in KDE 4 applications don't have icons by them.
There is just empty space. Some will sporadically have icons but not all.

Steps to Reproduce:
1. Try any KDE 4 app like KMail or Dolphin on Konversation

I am using Breeze Dark desktop theme, Breeze colors and icons, Plastik
decoration on Fedora 21 Plasma 5.1.
