I was having the exact same problem running Pulseaudio 2.0-6.1 on Debian 7.8 using Gnome 3.4.2. I was able to resolve the issue by installing the alsa-utils and alsa-base packages. I do not know the details as to why this worked as I am not a programmer. Hope this info is helpful!
